Position Overview:
We’re on the lookout for an enthusiastic, compassionate, and highly organised Support Coordinator to join our vibrant team. In this role, you will be the essential link between participants and the services they need, helping them navigate their NDIS plans to unlock their full potential. You’ll also be gaining valuable skills that will prepare you for a career in upcoming navigation services, helping people find their way through the service system.

Key Responsibilities:
- Build a connected life for people outside of funded support.
- Guide participants as they implement their NDIS plans, helping them understand options to make informed choices.
- Coordinating the implementation of these supports, ensuring capacity and funding management are optimised.
- Support participants to collect key evidence that supports their changing needs, and funding requests when required.
- Explore meaningful opportunities in recreation, employment, housing, and more to enrich their lives.
- Ensure mainstream services step up and fulfill their commitments.
- Providing regular, insightful reports to the NDIA and participants on progress and achievements.
- Achieve all relevant KPIs, including billable hours.
- Qualifications in social work, disability, mental health, or a related field.
- Experience in support coordination, case management, or similar.
- In-depth knowledge of the NDIS and its range of services.
- Confident in meeting KPIs including billable hours (highly regarded).
- Outstanding communication skills – you’ll be dealing with diverse participants, providers, and stakeholders.
- A natural problem-solver, multi-tasker, and decision-maker with a keen attention to detail.
- Eligible for or possess a Blue Card and NDIS Worker Screening Clearance.
