About the Role
As a Family Support Practitioner, you will play a critical role in providing outreach and in-home support to families, helping them build capacity, resilience, and stability. Working collaboratively with key stakeholders, you will provide tailored interventions to improve family functioning, strengthen parent-child relationships, and prevent children from entering out-of-home care.

Key Responsibilities
  • Provide intensive family support tailored to the needs of children, young people, and their families
  • Implement evidence-based interventions, including parenting strategies and practical life skills coaching
  • Support families to access community resources, education, and employment pathways
  • Conduct home visits and engage in early intervention strategies to improve safety and wellbeing outcomes
  • Work alongside Child Safety and other government and community stakeholders to deliver wrap-around support
  • Complete documentation and case notes in line with legislative and organisational requirements
About You
To thrive in this role, you will bring:
  • Experience in family support, child protection, out-of-home care, or community services
  • A strengths-based and trauma-informed approach to working with children and families
  • Strong skills in engagement, risk assessment, case management, and crisis intervention
  • A relevant Certificate IV, Diploma, or Degree in Child, Youth & Family Intervention, Community Services, Social Work, Psychology, or a related field (or working towards)
  • Excellent communication and relationship-building skills, with the ability to work both independently and as part of a team
  • High attention to detail for documentation and compliance
  • Blue Card for Working with Children and NDIS Worker Screening (or Yellow Card)
  • Current Driver’s Licence and the ability to travel for outreach work
